This Summer has seen the launch and ongoing development of the Basildon and Brentwood Care Navigation Partnership.
Commissioned by Basildon and Brentwood Clinical Commissioning Group, Public Health and Essex County Council, the scheme is a partnership with Basildon, Billericay and Wickford CVS, Community Agents and Provide’s Lifestyle Service. This exciting new scheme builds on the work already undertaken by the CVS’s Social Prescribing Navigator Service to increase the role of social prescribing in GP practices.
This is great news for patients and staff at practices in both Basildon and Brentwood. The Care Navigation scheme is intended to help people who have social as well as health needs, and would benefit from support with:
• Being helped to access community activities
• Lifestyle management
• Identifying volunteering opportunities in which they could play an active role
• Advice and guidance on issues like housing, welfare benefits and accessing statutory services
• Increasing personal resilience, boosting confidence and decreasing any feelings of social isolation
• Signposting to appropriate other services
Initially patients will be referred to Provide’s Single Point of Access and a one-to-one assessment of the client by a Care Navigator. This may be in the client’s own home, at their GP’s surgery or any other suitable venues agreed by the Care Navigator and client.
Referrals will come from GPs, self-referral and via other agencies which may be in contact with the client. This could include for example:
• Social work teams
• Other advisory agencies, charities or organisations
• Domiciliary Care
• Neighbourhood Teams
The Partnership will support the ability of patients and carers to access services, self-care, reduce social isolation and maintain independence. The Partnership will work with patients who require support and will utilise volunteers, and local voluntary and community organisations to enable patients to achieve their personal independence goals.
John Leslie, CCG’s Accountable Officer said, “the new model will join up three existing commissioned services. This is great as it will enable an expanded and consistent service across the South East neighbourhoods and districts”.
The Partnership builds upon encouraging findings identified by the Basildon and Brentwood Social Prescription service where positive patient feedback has been received.
